Topgolf Fundraiser - Registration/Donation Form

Don’t miss this afternoon of great food, fun, and friendly competition!

Please note that you can register up to 6 golfers for your group.  At check-in, the reservation will be under one name, whoever completes this "Donate via Pledge" form.

Join members of Northern Ohio Public Safety Support on Thursday, May 14th from 4PM - 7PM at Topgolf Cleveland, 5820 Rockside Woods Blvd N, Independence, OH 44131 and help support our mission: To provide essential on-scene support—at no cost—to public safety organizations during emergencies and training exercises, ensuring responders have access to rest, recovery, nourishment, and hydration.

The donation is $50 per player.  Up to 6 players and 2 spectators are allowed per golf bay. ("Support Pledge” options appear below your donation amount. Select a percentage, or choose “Other” to enter a custom amount, including “0”).

Your access to the Chairman’s Suite will feature:

  • Cheese and Charcuterie
  • Jumbo BBQ meatballs
  • Boneless BBQ and Buffalo wings with ranch dressing on the side
  • Spinach artichoke dip with tortilla chips
  • Cheesy macaroni bites
  • Fountain soda, iced tea, coffee, and water
  • Donut holes with chocolate and raspberry sauce
  • Cash bar available

Live automated leaderboards will track top scores from each guest and display them on your bay’s TVs, bringing all participants together for added excitement and friendly competition., which will include longest drive and closest to the pin contests.

  • Registration for 6 players and 2 spectators (a $400 value)
  • Advertising before and during the event
