Chatham-Kent Community Health Centres

The Chatham-Kent Community Health Centres are non-profit, community governed, multi-service health centres that provide primary health care, health promotion and community development services all under one roof. We are designed with the community in mind and for that reason we target marginalized populations such as the economically disadvantaged, individuals with addictions, individuals suffering from emotional or mental health challenges, individuals of First Nations descent, youth (13-21) and others. Our services can be accessed at four locations: Chatham, Wallaceburg, Walpole Island and Pain Court. We also collaborate with other community service agencies as we believe that by utilizing each other’s skills, we are able to deliver optimal programs that promote wellness and improved health for our clients and the community at-large. Many of our programs are accessible free of charge to all residents of the Chatham-Kent community.

The South and Central Health Foundation is a registered charity which strives to enhance the health programs and services of the Central Regional Health Authority throughout central Newfoundland & Labrador. We serve the following health facilities: Central Newfoundland Regional Health Ctr; Baie Verte Peninsula Health Ctr; A.M. Guy Memorial Health Ctr; Dr. Hugh Twomey Health Ctr; Connaigre Peninsula Community Health Ctr; Green Bay Health Ctr; Carmelite House, and Community Health Centres. It is growing to serve the medical needs of the town of Petawawa and area’s 16,000 people. The PCFHC mainly provides treatment and health education. As it grows, it will add illness prevention and health promotion programs.

California School-Based Health Alliance

The California School-Based Health Alliance (formerly the California School Health Centers Association) improves the health and academic success of children and youth by advancing health services in schools.  We envision a day when all of California's children and youth are healthy and achieving at their full potential. Since hiring our first paid staff in 2003, the Alliance has successfully grown to have an annual budget of more than $1,500,000 and thirteen staff.  We conduct policy work, promote the school based health center (SBHC) model, and assist SBHCs with program development to expand and strengthen school health services.  Our large network of collaborating partners includes 231 SBHCs, numerous school districts, federally qualified health centers and other providers, dozens of state and local policy organizations, and an e-communications network of more than 3,200 individuals. Our goal is to make school health centers an integral and sustainable part of the health care and educational systems. By putting health care where kids are – in schools – school health centers increase access to care and take health problems out of the classroom so that teachers can teach and students can learn.
