The Tanzania Health and Education Mission is a California non-profit, charitable organization established in 2007 in order to support the local health clinics and schools in the Kilimanjaro region of Tanzania. Our mission is to deliver, directly to the people of Tanzania, goods and services to improve their health and education. This includes: 1 - provide medicines and vitamins for rural dispensaries inTanzania, 2 - direct payment of scholarships for student, trainees and nursery school teachers in Tanzania, and 3 - purchase and distribution of food in three rural communities in Tanzania.

William Osler Health System Foundation is the fundraising partner of William Osler Health System. We receive gifts and bequests, primarily for medical equipment to enhance patient care and services in the communities of Etobicoke and Brampton Ontario. Our goal is to help the hospital provide the highest level of care to its patients.

Our goal is to create an innovative and sustainable bone and joint health care model that is patient-centered and significantly improves access, quality and efficiency; and to advance research and knowledge to ensure optimal patient care. Our work is dedicated to improving Albertans' health and wellness, regardless of location, age and social condition.

To assist our community by improving access to health and wellness, resources, and education. MCHF's goal is to build healthier communities through research and education. We will do this by assisting providers to improve the quality of care through evidenced-based process outcomes and add value to the community by providing for the individual. We seek to improve health outcomes, engagement and participation of care givers and the individual. We aim to provide tools, support and resources to empower patients to take a leadership role in their health by assisting in establishing care teams that demonstrate proactive rather than reactive patient care and improve workflow. We strive to build a cohesive system of a variety of services through programming to aid in navigating all the possibilities of an individual's health.

Life Health and Wellness Center was created to serve the homeless men, women and families. Our providers also serve Veterans and victims of Human Trafficking. Our clinic does not discriminate against race, color, sex, religion, sexual orientation, or ability to pay. We are a group of health care providers that provide family medicine to our patients. Many of our patients do not have health care but they do have chronic illnesses that have not been treated in years. We have contracts with the local universities and provide clinical education to students for Primary Care clinical rotations. The student are taught use evidence based methods to assess, diagnosis and treat patients and also educate patients about their current condition. We are also developing ways to sign patients up with insurance so they can become compliant with medications and therapies ordered to maintain positive health outcomes.

FCHS Foundation was established in 1987 as a charitable foundation to raise funds within the community to provide funding to Four Counties Health Services for enhancement of patient care. In addition to funding capital equipment costs, the Foundation supports the staff with funding for ongoing educational programs. Four Counties Health Services Foundation is currently raising funds to purchase new digital X-ray equipment and upgrade the current digital X-ray Suite.

Since being established in 2009, the Foundation has been able to purchase 4 pieces of necessary equipment for the Lady Dunn Health Centre. The Lady Dunn Health Centre services a catchment area of 5,700 people which includes the communities of Wawa, Hawk Junction, Missanabie, Michipicoten First Nation, Dubreuilville and White River. The equipment purchased by the Foundation enhances services to all those requiring services from the LDHC.

EKFH is located in the south east corner of British Columbia. The foundation presently stewards over 30+ funds in trust for hospital, health care facilities, residential care homes and programs guided by the Interior Health Authority. It serves the communities of Elkford, Sparwood, Fernie, Jaffray, Cranbrook, Kimberley, Invermere, Canal Flats, Radium, Golden and Creston. EKFH is a public charitable foundation and registered British Columbia Society. It is governed by a volunteer board of directors.

Over 20 million Canadians suffer from digestive disorders every year. We believes this is unnecessary and unacceptable. The CDHF reduces suffering and improvs quality of life by empowering Canadians with trusted, up to date, science-based information about digestive health and disease. We are the Foundation of the Canadian Association of Gastroenterology (CAG). As such, we work directly with leading physicians, scientists, and other healthcare professionals to help Canadians understand and take control of their digestive health with confidence and optimism. Through research and public education, we aim to: REDUCE the incidence and prevalence of digestive disorders; IMPROVE understanding of digestive health issues; SUPPORT those suffering from digestive disorders; and, ENHANCE quality of life for those living with digestive disorders.

The goal of the Misericordia Health Centre Foundation is to provide supplemental funding to meet the needs of a strong and meaningful healthcare system. Since its inception in 1983, the leadership and innovation of the Misericordia Health Centre Foundation has led to the development of vibrant, solid partnerships between the local corporate community and individual donors.
