The Foundation of Chatham-Kent Health Alliance is governed by a Board which consists of 12 volunteer directors. Since it’s inception in 1998, The Foundation of Chatham-Kent Health Alliance has lead activities formerly carried out by the Foundation of Public General Hospital, St. Joseph’s Hospital and Sydenham District Hospital. Through capital fundraising initiatives like 'Your Health, Your Future', 'The Future’s in Sight', 'Every Life Counts', Chatham-Kent Physician Recruitment Fundraising, 'Making the Right Investment - MRI Campaign' and ongoing capital campaign achievements, residents of Chatham-Kent have the ability to access leading-edge healthcare delivered within local modern facilities by highly-trained professionals. VHA Health and Home Support was founded in Ottawa in 1955. We are the largest not-for profit agency providing home support services in the city of Ottawa. VHA understands that home is where you want to be. We provide personal care services, and light housekeeping to help maintain the ill, frail or disabled individuals in the comfort of their own home. Our mission is to improve the health and quality of life for all people of the greater Pajaro Valley, especially the most disenfranchised. We do this by working “upstream” (before health problems occur or worsen) to help prevent future disease through education and disease prevention, as well as “downstream” (where problems require immediate intervention or treatment) to meet the most acute need for access to quality, culturally responsive services. In all our work, we collaborate with community members and agencies and rely on data to guide our decisions. In this way, we aim to pass on stronger systems and healthier lifestyles to future generations of Pajaro Valley residents.

Centre Grey is an extended community that includes the area from Feversham to Priceville, Dundalk to Chatsworth & numerous villages & municipalities in between. Our community also welcomes many seasonal residents & regular visitors who see Centre Grey as a home away from home. We all rely on Centre Grey Hospital in Markdale to provide first-rate health care; a task that it carries out admirably. In recent years, through the generosity of our community, the hospital has acquired a new ultrasound machine, colonoscope, vital signs monitor, operating room table & other essential medical equipment. Yearly equipment commitments are currently in excess of $150,000 and growing steadily. Your gift is critically important in helping maintain the capacity of our hospital to provide first-class health care to the Centre Grey community. With over 18,000 annual Emergency Room visits alone, you can be sure that your donation will help someone every day of the year.

Our Vision To instill the spirit of seva in doctors and the medical fraternity To provide a structured platform for medical and non-medical professionals, to volunteer their time and skills, and strengthen the healthcare system. Vision Statement: Every member of our North Coast community will have access to a seamless network of care and feel supported and empowered when facing a breast or gynecologic cancer concern. Mission Statement: Breast and GYN Health Project is a grassroots community resource for those facing breast or gynecologic cancer concerns. We promote healthy survivorship through information, hope, and healing support, while encouraging each person to become their own best medical advocate. We advocate within our community to address breast and gynecologic cancer concerns responsibly and holistically. We are cancer survivors and support persons healing through service and bearing witness to others.

We promote and assist change at the individual, community and social levels through the provision of quality services, community action and social advocacy. 1. Health and social services are provided directly to individuals and families to help them meet basic needs, learn new skills and enhance personal growth in an inclusive environment. 2. Through networking, planning and co-ordination with other community groups, and community development to support local community action groups, an attempt is made to resolve problems and issues affecting the community. 3. The Centre works at the local, provincial and national level in order to bring about social change that will benefit people living in the community served.

Nestled in a quiet area of western Toronto, the Harold & Grace Baker Centre is a 91-room Retirement Residence and a 120-bed Long Term Care Centre. The Retirement Residence provides an environment where residents enjoy a fulfilling, safe and happy life. The Long Term Care Centre is for people requiring daily nursing care. The Baker Centre is managed by Revera Inc., one of Canada’s leading providers of seniors’ accommodation, care and services and is overseen by a volunteer Board of Directors. As a not-for-profit organization, the Harold & Grace Baker Centre often has to rely on the generosity of others in order to replace aging equipment, undertake building maintenance projects and to be able to offer certain programs. The Baker Centre Health Care Foundation was established with a mission of raising funds for the Harold & Grace Baker Centre. A volunteer Board of Directors is responsible for overseeing the Foundation.
