Chebucto Connections (also known as Chebucto Community Development Association (CCDA)) is a charitable organization made up of local volunteers, that engages the community in making their neighbourhoods vibrant healthy places to live and work. We add value to community by helping others help themselves We have been helping people build connections to build community, with a focus on individual and community strenghts and assets, since 1985.

ALD Connect is a non-profit organization that brings together patients, families, physicians, scientists, advocates, and industry.Our mission is to improve health outcomes for patients with X-linked adrenoleukodystrophy (ALD) by empowering patients, raising awareness, and accelerating the translation of scientific advances into better clinical care.

Our mission is to strengthen the lives of individuals with disabilities by creating inclusive opportunities for community involvement, access to meaningful employment, and better options for supportive living.

We are people with Type 1 diabetes who share a vision: To create a culture of support and engagement in diabetes self-management through peer-based experiential diabetes education, physical activity and outdoor adventure pursuits. By creating a community that is motivating, supporting and inspiring individuals to take charge of their health today, we are helping to improve the quality of life for people with diabetes now and tomorrow.

The mission of SLC6A1 Connect is to fund medical research to improve patient outcomes related to SLC6A1 disease.

Kevin Gibbons and Carolyne Ariokot started Health Access Connect based on the need for antiretroviral treatment (ART) and other health services that they witnessed in fishing villages on Lake Victoria. Many people fail to get health services because it’s too expensive and difficult to travel to the nearest facility. We said, “There must be a way to address this problem!” HAC came from that initial need that we learned from the communities, and our programs are all designed to serve our mission to link Ugandans living in remote areas with healthcare resources. Over time we have worked with health workers to integrate more services, including malaria treatment, vaccinations, perinatal care, child checkups, and family planning.

Since 1993, we have been working to improve the quality of life in York Region by raising awareness through community education, and by providing support and access to dignified care for people living with and affected by HIV/AIDS. All our services are free of charge, confidential and anonymous. We envision an informed and compassionate society, which is supportive of people living with HIV/AIDS who are striving to overcome social, economic and service challenges, working with them towards a healthy and empowered lifestyle. The AIDS Committee of York Region recognizes: • The changing needs of people living with HIV/AIDS • The diverse communities affected by HIV/AIDS • The importance of providing relevant and responsive services in partnership with other service providers

We are committed to helping individuals with autism, their families and caregivers, as well as professionals, to access resources and services. These services will be provided in both official languages wherever possible. Care Connect International Inc., a 501(3) (C) corporation was formed in 2015, with aim to help provide medical attention to the less privileged, as well as help in obtaining life saving devices that can help them live long, meaningful and productive lives. Care Connect is geared towards ensuring excellent healthcare delivery that is community based and patient centered.

Connecting to Cure Crohn's and Colitis unites the IBD community to promote public awareness about Crohn's and colitis, to fund research for new treatments and cures, and to support patients and their families living with these chronic illnesses.

Hudson Pride Connections Center (HPCC) is a home and voice for the diverse LGBTQ community and our allies that advocates for our physical, mental, social and political well-being. We create safe and vibrant spaces to gather and celebrate our lives. HPCC is a 501(c)(3) non-profit, non-governmental, community organization situated in the Journal Square neighborhood in Jersey City, one of the most vibrant and diverse cities in this country and home to the largest LGBTQ community in New Jersey. Hudson Pride was established in 1993 to serve as an advocate for both the LGBTQ and HIV/AIDS communities in Hudson County and more than 20 years later, we are still delivering a wide array of services, programs and events to meet their on-going needs. Our programs and services include LGBTQ support groups for youth, young adults, middle adults and seniors, specialized groups for HIV+ gay men, transgender individuals and women of color, community training and education focused on helping provider agencies to deliver LGBT and HIV competent services, LGBTQ health education, HIV outreach services, social service linkages and advocacy.
