The Delhi Community Health Centre provides a range of health services delivered by physicians, allied health professionals, and community agencies. These include: - The Delhi Family Health Team: a team of family physicians, nurse practitioners, registered nurses, social workers, a mental health worker, pharmacist, and other professionals who work together to provide health care for their community - Independent physicians through the Norfolk Family Health Group providing comprehensive primary health care services to their enrolled patients both during regular office hours and through an after-hours service - A walk-in medical clinic

Although access to contraception and abortion was decriminalized over 40 years ago, barriers continue to exist to comprehensive sexual health in many communities across Canada. People of all ages, but especially young people, struggle with cost, lack of confidentiality in small or rural communities, and insufficient access to reliable, nonjudgmental information. CFSH’s donors are helping us provide the reliable and non-judgemental sexual health information all Canadians deserve. Your donations help us develop and distribute sexual health promotional materials, inclusive sexual health education materials, and accessible social media toolkits for our member organizations. In addition your support allows us to facilitate communication between our incredible member organizations to ensure we have a strong and united national voice for sexual and reproductive health.
The Foundation's current focus is raising funds needed for construction, furnishing and equipment for a new health centre.
Access Alliance works to promote health, wellbeing and improved access to services for immigrants and refugees in Toronto. From a location in downtown Toronto and our ACCESSPOINT locations in the east and west ends of the city, we provide primary health care, community programs, settlement services, and LGBTQ- and youth-focused programming to help newcomers, refugees, immigrants and their families in Toronto address medical, social, economic, environmental, settlement and integration issues. We are a leader in community-based research and advocacy on issues related to the determinants of health and wellbeing and increasing access to services for marginalized groups. We actively seek to break down silos and build linkages with other agencies to increase access to the services needed by immigrants and refugees. Access Alliance works toward a vision of the future in which diverse individuals, families and communities can achieve health with dignity.

A strong community needs great health care close to home. Thanks to the generosity of donors, the Foundation supports the Health Centre’s remarkable physicians and staff as they take care of our community. Donations support facility renovations to our patient care areas. For example, support from donors helped build a new wing that opened in 2012. Donors also help purchase essential medical equipment, like a new endo-bronchial ultrasound that pinpoints lung cancer, and funds training for our dedicated health care professionals. Current priorities include renovating our patient spaces like lounges and waiting areas to make the more comfortable for patients and families; building a new Dialysis Centre; and building and equipping two new Interventional Suites in our Diagnostic Imaging department.
The Health and Home Care Society of BC is an independent and BC-based not-for-profit organization formerly known as the Victorian Order of Nurses (VON) BC. Since 1898, we have been taking care of British Columbians with healthcare initiatives tailored to the needs of the communities we serve. We continue this tradition of caring today by offering preventive and supportive health services to keep people healthy and in the comfort of their own homes and communities. We offer charitable programs to support seniors, caregivers and other individuals in need: -Meals on Wheels delivers more than 500 meals every weekday to homebound Vancouver-Richmond residents, as well as visits from our friendly volunteers and an informal check on their safety. -the Family Respite Centre is a safe, home-like environment where seniors benefit from temporary nursing care and recreational and therapeutic activities with their peers, allowing caregivers to get much-needed rest from their responsibilities.
Founded in 1993, the Health Services Foundation of the South Shore resulted from a merger of the South Shore Regional Hospital Foundation and the Fishermen's Memorial Hospital Society. According to it's by-laws, the Foundation's purpose, is to "raise money in an orderly, continuing, and coordinated way for the South Shore Regional Hospital and Fishermen's Memorial Hospital." The Foundation responds to the needs of the hospital, but is accountable to the donors and the community, whose gifts make its work possible. Many gifts are designated to a specific program or service. Assuring that the wishes of the donor are carried out, is a responsibility the Foundation takes very seriously. The affairs of the Foundation are guided by a Board of Directors, composed of volunteers with diverse interests and expertise, from various county communities.

Working in partnership with other community and city-wide organizations, we provide support and a wide range of flexible, responsive and barrier-free services to help our participants effectively manage the issues, concerns and problems they are facing.
