Our mission is to reduce health disparities in minority communities with our unique approach to patient care and community involvement. In minority communities, many patients often don’t seek care or clash with the current system because of cultural differences. We work to integrate patients on the fringe of the healthcare system through culturally sensitive health literacy campaigns, community-based health screenings and our culturally conscious health education television program. It is only through partnership with the communities in which we serve that true transformation will occur and disparities can be reduced. We believe that it is possible for everyone to obtain the same quality care and our mission is centered on making this goal a reality."

HDSA's network of chapters, affiliates, HDSA Centers of Excellence, social workers and support groups provides a seamless connection for help, education and outreach to HD families and health care professionals across the United States.

Guide Dogs for the Blind provides enhanced mobility to qualified individuals through partnership with dogs whose unique skills are developed and nurtured by dedicated volunteers and a professional staff. Established in 1942, Guide Dogs for the Blind continues its dedication to quality student training services and extensive follow-up support for graduates. Our programs are made possible through the teamwork of staff, volunteers and generous donors. Services are provided to students from the United States and Canada at no cost to them.

Caring Voice Coalition is dedicated to building relationships with charitable organizations founded to help individuals and families affected by serious chronic disorders and diseases. Caring Voice will work together with these organizations to offer outreach and support services that directly benefit the patient community.

The Chicago Hispanic Health Coalition’s mission is to promote healthy behavior and prevent chronic disease and health disparities in the Hispanic communities of metropolitan Chicago. CHHC improves health in Hispanic communities by sponsoring culturally appropriate, evidence-based health education classes, and by providing a forum for policy development.

The DBCC works to improve breast health in Delaware by increasing awareness, expanding access to screening and care, and supporting patients and survivors through outreach, education, patient navigation, peer mentoring, and financial assistance.
