To empower community-members with the resources they need to thrive while building community, fostering connection and inspiring a lifestyle of service. We serve individuals and families in Petaluma, California and rural parts of the Dominican Republic.

Oak City Cares is a public-private partnership that exists to support the self-determination of our neighbors experiencing homelessness through trusting relationships and connections to coordinated services that increase their ability to be safely and stably housed.

IFC confronts the causes and responds to the effects of poverty in our community. We believe in a community where everyone's basic needs are met, including dignified and affordable housing, an abundance of healthy food, and meaningful social connection.

The So Big Mountain House is a Christ-centered maternity home that provides food and shelter for pregnant women and their children and connects each woman with resources to help her develop a better tomorrow for herself and her family.

Citrus Heights HART is a volunteer-run community effort to end homelessness in the city of Citrus Heights. Our goal is to act as a resource forthose facing extreme poverty and chronic instability by connecting them to local services.

Connections and You, Inc. (CYI), a 501(c)3 non-profit organization, was established in October of 2008 with a desire to help young men become better people in our society. CYI provides a very comprehensive mentoring program called, The Distinguished Gentlemen's Club. We believe that although manners takes an individual a long way, a positive character will take them further. In this program we concentrate on establishing and building positive character into these young men, which would ultimately cause these GENTLEMEN to be DISTINGUISHED, in their schools, in their community, in their families, and in society. Our mission is to connect caring adults with children that would assist in cultivating and building positive character in them through intentional mentoring. Families Moving Forward helps families in the crisis of homelessness find their way home. We provide temporary housing, case management, skills education, and connection to community resources to help parents and children thrive in stable homes.

The Relief Bus exists to respond to god's heart for the poor and broken by going to the streets to offer food, compassion, and connections to resources that lead to life transformation. To accomplish this mission we partner together with volunteers and care giving organizations.

The Arc Montgomery County connects people of all ages and abilities with their communities to build inclusive and fulfilling lives. We provide daily support services to nearly 1,100 children, youth, adults, and seniors, with and without disabilities, and their families.

JCHE provides safe and affordable independent housing where older adults of all backgrounds can age in community. Our programs and services enable residents to live a full life of connection and purpose in a dynamic and supportive environment.

Hope Community creates connections that strengthen the power of community members and communities. We cultivate community leaders, build community capacity, care for the housing and community spaces we develop, and pursue equity and diversity in all we do.
