Community Bridges envisions a thriving community where every person has the opportunity to unleash their full potential. Our family of ten programs delivers essential services, provides equitable access to resources, and advocates for health and dignity across every stage of life. Every year, we meet the needs of thousands of local children, families and seniors with broad support from individuals, businesses, foundations and government. We believe that when we work together, anything is possible. Our Child Development Division's experienced, bilingual teachers offer early education opportunities, developmental screenings, and family engagement at six centers throughout the county. Child & Adult Care Food Program is a nutrition assistance program that ensures low-income children and adults attending licensed facilities receive healthy meals and each every day. Elderday Adult Day Health Care is the only program of its kind in Santa Cruz County, serving approximately 70 elders and adults with physical, emotional, or cognitive impairments each day. It provides the services this vulnerable population needs to stay healthy and maintain the highest level of independence possible. Our Family Resource Collective works with individuals and families to build safe, healthy, and compassionate communities through education, resources, and advocacy. It includes La Manzana Community Resources, Live Oak Family Resource Center, Mountain Community Resources, and Nueva Vista. Lift Line provides accessible rides to low-income, disabled, elderly and frail individuals each year, ensuring that their medical, nutritional, and emotional needs are met. Meals on Wheels for Santa Cruz County serves more than 600 meals per day to low-income, frail, elderly, and disabled residents. With no required payment or waiting list, those in need can count on a warm meal at one of our five centers, or delivered straight to their doors. Women, Infants & Children (WIC) provides nutrition education, food stamps, and breastfeeding support to more than 9,000 low-income pregnant women, infants and children each month, helping families thrive.

The Little Sisters of the Poor are an international congregation of Roman Catholic women religious founded in 1839 by Saint Jeanne Jugan. Together with a diverse network of collaborators, we serve the elderly poor in over 30 countries around the world. Continuing the work of Saint Jeanne Jugan, our MISSION is to offer the neediest elderly of every race and religion a home where they will be welcomed as Christ, cared for as family and accompanied with dignity until God calls them to himself.

Wheels 4 Life is a non-profit charity (501c(3)), that provides bicycles to people in great need of transportation throughout the developing world. They work with volunteers in the field, local organizations and non profits, schools, community leaders and health care clinics they help them identify those people in genuine need of a bike. These people are those that live in both primative and remote areas, with no access to, nor the means of affording public transportation. The closest school, doctor, or work place might be as far as 10 to 20 miles away.The gift of mobility can make all the difference to a person’s life and help them to break out of the vicious cycle of poverty.

Patients come long distances to benefit from techniques that Lahey surgeons have pioneered. The Neurovascular Institute is among the most advanced in the region for the treatment of strokes and brain aneurysms. The Liver Transplantation Team was the first in New England to perform adult living-donor transplant procedures. And surgeons are using minimally invasive laparoscopic methods, drastically reducing the recovery time following surgery for diseases such as bladder cancer.Lahey Clinic Medical Center (LCMC) in Burlington encompasses an ambulatory care center serving more than 3,000 patients each day and a 259-bed hospital. Lahey Clinic Northshore in Peabody serves more than 400 outpatients each day and includes a 10-bed hospital. Both feature 24-hour emergency departments, with a Level II Trauma Center based at LCMC.Lahey's community-based physicians have a wide range of special interests. Many of the community group practices feature adult internists, pediatricians, and family practice specialists. Lahey Clinic Medical Center is a teaching hospital for Tufts University School of Medicine. In addition, many of our physicians hold teaching assignments at Harvard Medical School and Boston University School of Medicine. The Clinic maintains residency and fellowship programs for more than 100 new physicians in 19 specialties.As a research center, Lahey Clinic offer patients access to clinical trials of new therapies for diseases such as diabetes, prostate cancer, heart disease, and cataracts. Research programs at Lahey Clinic encompass more than 200 clinical trial protocols and participation in numerous national and international studies.
