Our Mission

At Heart to Heart Hospice Foundation we believe it is our responsibility to promote and provide quality of life for those with life-limiting conditions, and the family members who are caring for them. We carry out this mission by providing supportive programs to those with the greatest un-met need in the communities that we work in, including Veterans, patients suffering from Alzheimer’s disease and other forms of dementia, and those living with financial hardship. We also provide educational opportunities and scholarships to healthcare providers and to community members, as well as long-term bereavement support, because we believe that this increases the impact of our mission.
