The Alex is a not-for-profit organization that has been in existence since 1973. The Alex provides services and programs to individuals and families who experience barriers to health. Using a multidisciplinary team approach, we focus care on assessment, intervention and promotion of all aspects of health. We embrace a model of healthcare that is accessible, responsive and participatory. The Alex has grown to now include 6 major programs at 3 different locations. Services and programs are provided at the Family Health Centre, the Seniors' Health Centre, the Community Health Bus, Pathways to Housing, the Youth Health Centre and HomeBase. The mission of Addison County Home Health & Hospice, Inc., a non-profit agency, is to provide high quality, comprehensive community health care to residents of Addison County.

We are known as innovative, creative and supportive of a worker - involved approach to workplace health and safety. The work of the Alberta Workers' Health Centre will be accomplished, alone or with others, by: • acting as a ‘first contact’ point for workers seeking help; • providing workers with education and information to help them solve problems; • working to promote health and safety issues in high schools and other learning institutions; • raising public awareness of workplace health and safety issues.

The South End Community Health Center is a board governed non-profit, comprehensive health care organization for all residents of the South End and the surrounding communities. In the Centre: At the GRCHC, we have a team of health professionals dedicated to client-centred care. This includes physicians, nurse practitioners, nurses, a social worker, registered dietitian, counsellor, chronic disease specialist, and community support facilitator/advocate. Our registered clients have access to this team who work with them towards helping them achieve better health and wellness. In the community: Our health promotion and community development team works with dozens of community partners and agencies to reduce barriers and improve health & wellness.

Masonicare Home Health and Hospice, Inc. provides home health and hospice services to clients throughout much of the state of Connecticut. Masonicare Home Health and Hospice, comprised of a team of highly qualified and caring individuals, is dedicated to excellence in home health and hospice care. We continually strive to improve the health and wellness of patients and families throughout Connecticut with our responsive, accessible and quality-driven approach to care at home and in the community. Health Care for the Homeless works to prevent and end homelessness for vulnerable individuals and families by providing quality, integrated health care and promoting access to affordable housing and sustainable incomes through direct service, advocacy and community engagement.

The BRHC Foundation supports and augments health care for all who are served by the Brandon Regional Health Centre through funds received from personal donations, memorial gifts, gifts of planned giving and special occasion gifts.
