The Milwaukee Area Health Education Center (Milwaukee-AHEC) connects students to careers, professionals to communities, and communities to better health. Since 1992, Milwaukee-AHEC has worked to forge collaborative partnerships with health-related employers, community-based organizations, academic institutions, and other sectors to advance community and public health that will impact health outcomes.

The San Francisco Public Health Foundation develops and mobilizes resources in support of the goals of the San Francisco Department of Public Health and its community partners to protect and promote health.

Harvard Street Neighborhood Health Center, Inc. (HSNHC) is a Federally Qualified Health Center providing health care without regard to race, sex, color, national origin, or ability to pay. HSNHC addresses the physical and behavioral health needs of families and children living primarily in the Dorchester, Mattapan, and Roxbury neighborhoods of Boston. The Health Center serves as a primary source for health education and works with the community to identify and put into action needed health services and works with community groups to ensure the well-being of the communities it serves. It is HSNHC's core belief that high quality health care delivered in a dignified manner should be made available, accessible, and affordable to all people as a right, rather than a privilege.

The California School-Based Health Alliance (formerly the California School Health Centers Association) improves the health and academic success of children and youth by advancing health services in schools.  We envision a day when all of California's children and youth are healthy and achieving at their full potential. Since hiring our first paid staff in 2003, the Alliance has successfully grown to have an annual budget of more than $1,500,000 and thirteen staff.  We conduct policy work, promote the school based health center (SBHC) model, and assist SBHCs with program development to expand and strengthen school health services.  Our large network of collaborating partners includes 231 SBHCs, numerous school districts, federally qualified health centers and other providers, dozens of state and local policy organizations, and an e-communications network of more than 3,200 individuals. Our goal is to make school health centers an integral and sustainable part of the health care and educational systems. By putting health care where kids are – in schools – school health centers increase access to care and take health problems out of the classroom so that teachers can teach and students can learn.

The Alex is a not-for-profit organization that has been in existence since 1973. The Alex provides services and programs to individuals and families who experience barriers to health. Using a multidisciplinary team approach, we focus care on assessment, intervention and promotion of all aspects of health. We embrace a model of healthcare that is accessible, responsive and participatory. The Alex has grown to now include 6 major programs at 3 different locations. Services and programs are provided at the Family Health Centre, the Seniors' Health Centre, the Community Health Bus, Pathways to Housing, the Youth Health Centre and HomeBase. The mission of Addison County Home Health & Hospice, Inc., a non-profit agency, is to provide high quality, comprehensive community health care to residents of Addison County.

Family Health Center of Worcester is dedicated to improving the health and well-being of all residents in the Greater Worcester area, especially culturally diverse populations, by providing access to affordable, high quality, integrated, comprehensive, and respectful primary health care and social services, regardless of patients’ ability to pay.

We are known as innovative, creative and supportive of a worker - involved approach to workplace health and safety. The work of the Alberta Workers' Health Centre will be accomplished, alone or with others, by: • acting as a ‘first contact’ point for workers seeking help; • providing workers with education and information to help them solve problems; • working to promote health and safety issues in high schools and other learning institutions; • raising public awareness of workplace health and safety issues.

The South End Community Health Center is a board governed non-profit, comprehensive health care organization for all residents of the South End and the surrounding communities. We are committed to providing the highest quality, culturally and linguistically sensitive, coordinated health care and social services to every patient, regardless of their ability to pay.

In the Centre: At the GRCHC, we have a team of health professionals dedicated to client-centred care. This includes physicians, nurse practitioners, nurses, a social worker, registered dietitian, counsellor, chronic disease specialist, and community support facilitator/advocate. Our registered clients have access to this team who work with them towards helping them achieve better health and wellness. In the community: Our health promotion and community development team works with dozens of community partners and agencies to reduce barriers and improve health & wellness.
