Upstream provides a choice of ongoing flexible support services, via generic mental health case management and family focused case management models. In addition, limited permanent affordable housing options are available to individuals afflicted with serious and persistent mental illness in order to assist with optimum functioning in the community. Upstream provides ongoing recreational group activities to its consumers which are designed to decrease stigmatization and increase social networking opportunities. Emphasis is placed on reducing isolation, increasing educational/vocational opportunities, activities of daily living. Currently, a Youth Mental Health Case Management Program is being internally funded based on feedback from the community and our success with this population during the pilot phase of the initiative. The majority of our fundraising efforts are aimed at ensuring the fiscal viability of this successful Youth Program.
