The North York Family Health Team (NYFHT)became fully operational in 2008. It is composed of administrative staff, nurses, social workers, dietitians, nurse practitioners and 66 family physicians. There are approximately 64,000 patients with the FHT.
NYFHT is an academic Family Health Team associated with the Department of Family & Community Medicine at North York General Hospital and the University of Toronto.
The NYFHT serves the diverse population of North York by providing comprehensive primary healthcare, developing programs focused on health promotion, disease prevention, chronic disease management in collaboration with community partners, and creating multidisciplinary teams of healthcare providers to deliver and coordinate high quality care.
The NYFHT also plays an active role in education, research, patient care, policy development,inter-professional and primary care learning, disease prevention, and chronic disease management.
