OUR SERVICES HAVE BEEN EXPANDED FROM THE FAIRFAX COMMUNITY TO THE SURROUNDING COMMUNITIES OF WESTERN FAIRFAX COUNTY AND LOUDOUN COUNTY WITH THE OPENING IN MARCH 2021 OF THE CHANTILLY OUTPATIENT CENTER.
MEALS ON WHEELS OF SUMMERVILLE INC IS A COMMUNITY SPONSORED PROGRAM. ITS PURPOSE IS TO PROVIDE HOME DELIVERED NUTRITIOUS NOON MEALS SERVICE FOR THE ELDERLY, OR OTHERS WHO ARE HOMEBOUND, ISOLATED, OR INCAPACITATED TO THE EXTENT THAT GOOD FOOD PREPARATION IS NOT POSSIBLE.
Palliative & Supportive Care of Nantucket Foundation is dedicated to increasing awareness and educating the community about palliative and supportive care issues as well as to funding all of the Palliative & Supportive Care Program's services and initiatives, all of which are provided to the community free-of-charge.

At LifePath Hospice, we go way beyond those basic hospice requirements to identify and meet the specific needs of each patient and family—from extra homelike touches in our two hospice houses to the hands-on bereavement care we provide to families long after their loved one has died. The mission of LifePath Hospice is to provide quality palliative care and relieve the suffering of those in our communities affected by life-limiting illnesses and end-of-life issues, maintaining the highest ethical standards, so all may live as fully and comfortably as possible.
Our Mission is to assure the promise of hospice. We do this by increasing access to hospice care, helping communities understand the truth of hospice, and providing financial assistance to benefit hospice patients and their caregivers.
Mama’s Kitchen provides medically tailored, home-delivered meals and nutrition education at no cost to San Diego residents who are at risk of malnutrition due to critical illnesses (such as HIV, cancer, heart failure, diabetes, and kidney disease). Their goal is to improve client health and dignity by ensuring access to appropriate, nutrition-focused food and related services.
Their mission is to transport America's veterans to Washington, D.C. to visit the memorials dedicated to honoring those who have served and sacrificed for the country.
Our mission is to provide comfort care at the end of life. We believe that a life fully lived, no matter how long, should include an end-of-life experience of comfort, dignity, and peace. Our goal is to keep each patient as comfortable and independent as possible, while supporting the emotional and spiritual needs of our patients and their loved ones. We were the first non-profit Hospice in the area and one of the first 100 Hospices in the nation. Our founders were a group of dedicated doctors, nurses and community volunteers that began providing care to the dying on their own time. Our organization began in 1979 before there was insurance reimbursement for Hospice care and this dedication to our patients remains our culture to this day. Ionia Area Hospice approached Hospice of Lansing joined forces and a formal partnership was formed in 1984. Should the time come when it is no longer possible for a patient to remain at home, Stoneleigh Residence is a unique, comfortable, peaceful, and home-like alternative. Surrounded by gardens, a cascading pond, trees and wildlife, the Residence is designed to take full advantage of the natural beauty of its wooded 20-acre site. We are a true community-based Hospice, created out of necessity and driven by our mission to serve all by bringing kindness, respect and quality to the end-of-life. Hospice of Lansing, Stoneleigh Residence and Ionia Area Hospice provide pain management and personal care, respite care, emotional and spiritual support for patients, volunteer support and grief support for family members after the loss.
